Continuing care plan has to be drawn by the client with the assistance of the Counsellor which are set plan of action developed before discharge or transfer to another level of care. They are structured, goal-oriented list of services based on recovery capitals available. The plan also includes continuation of the disulphiram or Antabuse prescribed by the Doctor and regular follow up visits, attendance of the Mutual help programs, etc. The goals of the continuing care is to:
- Sustain abstinence
- Develop continuing recovery supports
- Gain community living
- Gain employment skills
- Gain education
- Obtain employment or schooling
- Obtain counseling for co-occurring mental disorders
- Develop a deeper understanding of self and others
- Increase responsibility
- Work on resolving family difficulties
- Consolidate, reinforce, and become comfortable with life changes
- Integration into the community with a meaningful role

- Continued work on life skills, such as solving problems
- Relapse prevention training
- Health and wellness
- Education and career planning
- Supportive counseling
